Early on, Dovetail realized there was a gap in the healthcare system - and we could address it. Through partnerships with providers and health plans, and caring individual relationships, Dovetail is helping patients manage their health at home while avoiding unnecessary and costly hospital admissions.

Dovetail is not a traditional home care service. We develop strong connections with our partners, and we build caring relationships with the individuals we serve. Our programs give us the flexibility to adapt to each particular situation. In every case, we offer highly-qualified, compassionate professionals who can provide medication assessment and management, health assessment, transition-specific services, health monitoring and coaching, and additional health management measures as needed.
