The Clinical Pharmacist provides high quality, personalized in-home medication reconciliation and optimization as well as health management services with the focus of preventing admissions, readmissions and adverse events. Key responsibilities include in-home medication assessments, creation of a medication management plan with supporting documents, and development of an on-going health management plan. CGP preferred.
