Nurse Care Manager
Dovetail's Nurse Care Manager manages the care of home-based aging adults, providing assessment, support and monitoring vital data via home telemonitoring devices. Our nurses use an interdisciplinary approach to patient care management by coordinating services with other clinical staff and outside providers.
