While it is not possible to prevent every admission to a hospital or rehab facility, at Dovetail Health we do believe that we can help to return patients to their homes sooner, and provide the support they need to stay there. Key to delivering a return on investment is selecting the patients who are at greatest risk of readmission. Dovetail uses proprietary intervention optimization technology based on rules, patterns, claims, medical history, provider feedback and historic program outcomes to identify the most appropriate patients.

• Multiple Hospitalizations / SNF Admissions
• Multiple medications & dosage changes
• One or more chronic diseases such as diabetes, CHF, CAD, COPD, etc.
• Aging syndromes, such as falls, mild cognitive impairment, and functional decline
• Concerned family and caregivers who need support at home
• Patients going home or to an ALF
