Dovetail Health's services for individuals are tailored to each client's personal needs. We begin by offering two different programs, each of which can then be customized to address the specific challenges you face.

TRANSITION SERVICES
Ideal for individuals who are being discharged from a short-term visit in a hospital or rehabilitation facility, this 30-day program helps reduce preventable readmissions by offering:

  • In-home visit with a Pharmacist following discharge
  • Pharmacist review and reconciliation of medications, from pre-hospitalization to post-rehab — and everything in between
  • Health coaching and support
  • Preparation for upcoming healthcare provider visits, including assistance with medication lists and questions to ask your doctor
  • Regular check-in phone calls
  • Recommendations and guidance for additional resources such as transportation, home services, and community wellness programs

COMPREHENSIVE HEALTH AND MEDICATION MANAGEMENT
For individuals who want to remain at home yet need some extra ongoing care and personal support in order to maintain their health and safety, this program offers:

  • Initial 30-day in-home assessment period to identify recommendations and next steps
  • Assessment and reconciliation of medications, and assistance with medication regimen
  • Additional transition services if needed
  • Daily chronic illness coaching and/or monitoring
  • Caring, personalized support for ongoing care needs from your Dovetail care team, including a Pharmacist, Nurse Care Manager, and Personal Care Coordinator
  • Regular check-in phone calls and home visits as needed
  • Assistance preparing for upcoming physician visits, including the collection of health trend and medication data
  • Daily reminders, referral management, and help coordinating other types of care as needed
  • Remote monitoring of vital signs (to catch small changes in condition and prevent an acute event), as appropriate
