Your Care Team

Each person we work with has unique needs, and we respond to these needs with personalized, custom services. As a Dovetail client, you will work closely with your own dedicated team of professionals, including:

The Nurse Care Manager (a Registered Nurse, or “RN”) performs an initial in-home assessment, recommends a health management plan based on the client's conditions and needs, measures the client's trends over time via remote monitoring, and calls weekly to check in. As the heart of the team, the RN develops a trusting, personal relationship with the client and regularly visits the home for additional follow-up checks.

The Licensed Pharmacist visits the client at home for a thorough initial medication assessment, helps the client understand how to obtain and administer medications correctly, and monitors all of the client's prescriptions and over-the-counter medications, sharing any discrepancies or concerns with the client's physicians. Available by phone and for in-home visits as necessary, the pharmacist maintains an ongoing relationship with the client.

The Personal Care Coordinator is available to the client by phone at all times, helping to coordinate all of Dovetail's services while also providing referrals for any additional health and lifestyle services that may contribute to the client's ability to live safely and well at home. The care coordinator often helps clients prepare for medical appointments and participates in regular check-in, reminder, or companion calls as needed.
