Dovetail will ensure that each patient receives the support they need from a caring, dedicated clinical team. Chronic diseases, complicated medications, and returning home from the hospital can all be overwhelming; at Dovetail, our goal is to ensure that they remain safe at home by identifying and addressing the most common risks.

Every Dovetail patient receives:

• Access to a dedicated clinical team, including a registered nurse, pharmacist and care coordinator
• Regular in-home assessments and telephonic check-up calls
• Customized health and medication reports
• Patient education materials (Healthwise for Life Handbook)
• Care coordination and individual support as required

Dovetail is not the VNA, or a home health agency. We do not replace existing physician or pharmacy relationships. In fact, Dovetail works closely with all other caregivers involved to ensure that all care needs are communicated and coordinated.
