Dovetail Health works as an extension of physician practices, acute and sub-acute care facilities. Our goal is to meet patients where they live to review medications, ensure adherence, and coordinate care. Ultimately we simplify and improve the management of complex patients. We coordinate closely with provders to implement appropriate in-home services to patients coping with chronic illnesses, complicated medication regimens, and/or a recent discharge from a care facility. With the added support we offer, patients can avoid exacerbating their conditions or mismanaging their medications — and our monitoring services help them track trends in their health. As a result, providers will experience more efficient patient appointments, reduced costs, and — most important — better patient outcomes.
