Dovetail Health works as a home-based extension of your practice. Our goal is to review medications, ensure adherence, and coordinate care, ultimately simplifying and improving your interactions with complex patients. We coordinate closely with you to provide appropriate in-home care management services to patients coping with chronic illnesses, complicated medication regimens, and/or a recent discharge from a care facility.
