Dovetail Health's goal is to partner closely with each provider to determine how we might best help their patients. With our turnkey in-home support and services patients can be happier, healthier, and better informed about their health conditions.

• In addition to helping patients live safer, healthier lives at home, we often help patients monitor their vital health data, preventing emergencies and enabling them to present their providers with more in-depth information about their conditions.
• We help make patient visits more effective by arming patients with critical information (including personal health data and medication charts), helping patients organize and present their concerns, and empowering providers to make informed decisions about care management.
• We customize a communication plan for each provider, to ensure that we transfer the information needed — when it is most appropriate — so that we are reducing provider burden, not adding to it.
• Our experience has proven that patients will experience greater medical stability and improved outcomes, which can reduce emergency admissions and hospitalizations while helping providers manage patients within budget and meet quality care performance measures being introduced by insurers.
